*1　“Permission Form” in the application form for entrance examination has been deleted on August 24, 2023 (you do not have to submit this form).

• In order to read the PDF files listed above, kindly download/update the latest Adobe Acrobat Reader.
  Since auto-form is adopted in the allocation forms above, if you fill in the information from Adobe Acrobat Reader, some items such as physical address and name are reflected to specific spaces automatically.
• In case that the information would not be expressed properly, kindly hand-write required information.
• In case that you print out the documents, please print out with A4 paper.
• Do not use smart phone or tablet device to fill in the information. There is possibility that information would not be expressed properly.

Request of Past Exam Paper of Graduate Programs (*This service is for the person who has physical address in Japan only, and the exam paper is also Japanese version only)

Applicant can receive the latest past exam paper (FY 2021) i) at the Department of Student Affairs, Office for Division of Health Sciences directly or ii) by mail (NOT e-mail)

In case that you request sending it by mail, prepare an blank envelope for reply with pasting a stamp of 94 yen and write your physical address in Japan on it, and send it to the address below:

*DO NOT forget to choose your requesting course from D to G below and make sure to write one of them on the envelope:

D: Course of Nursing Science, Doctoral Program (First Semester);
