We have added the Japanese text “（出願資格(6)で出願する者のみ）” to Application Form for Entrance Examination of Doctoral Program, p.8 a” Check List of Application Documents (Doctor Course) at No.14”. Only Japanese Text has been added no change for English part.

How to request related documents

Request of Past Exam Paper of Graduate Programs (*This service is for the person who has physical address in Japan only, and the exam paper is also Japanese version only)

Applicant can receive the latest past exam paper (FY 2021) i) at the Department of Student Affairs, Office for Division of Health Sciences directly or ii) by mail (NOT e-mail)

In case that you request sending it by mail, prepare an blank envelope for reply with pasting a stamp of 94 yen and write your physical address in Japan on it, and send it to the address below:

*DO NOT forget to choose your requesting course from D to G below and make sure to write one of them on the envelope:
